Clinical Social Worker II
The Clinical Social Worker II provides social work services to patients and their families to assist in meeting psychosocial and clinical needs. Utilizing expertise and experience, this role identifies and addresses the needs of patients both inside and outside the clinical setting through therapeutic and other clinical techniques.
Patient Focus: Determines which patients require clinical social work efforts to ensure appropriate support and environment for staff and patients.Information Analysis: Analyzes information from patients and clinical staff to understand the patient's situation and communicates this effectively to prevent issues arising from information gaps.Resource Linkage: Connects patients with community resources and health opportunities to ensure they receive high-quality care.Care Planning: Develops care plans for patients with minimal assistance to facilitate appropriate treatment.Education and Liaison: Educates staff, patients, and families on critical information related to care, serving as a liaison to enhance understanding.Crisis Support: Provides crisis support and leadership in high-tension situations, using clinical expertise to de-escalate and support all parties involved.Psychosocial Services: Completes psychosocial assessments and links patients with suitable care options based on their medical and social status.Counseling Techniques: Utilizes various counseling and therapy techniques to help patients and families feel at ease regarding their clinical and medical situations.Additional Duties: Performs other duties as assigned.Minimum Qualifications
- Education: Master's of Social Work (MSW)
- Experience: Minimum of two (2) years of recent experience in a health-related environment
- Licensure: Licensed Clinical Social Worker (LCSW)
