Social Worker - Kids Path

Authora Care Collective is recruiting a full‑time hybrid Social Worker in the Kids Path program. The position covers the Greensboro and Burlington campuses and serves the entire Authoracare catchment area.

Monday–Friday, 08:00 am – 05:00 pm (hybrid). Travel required within service area.

• Conduct comprehensive assessment of the social, emotional, and practical needs of pediatric patients and families.
• Facilitate resolution of psychosocial/emotional conflicts that may interfere with patient and family benefit.
• Develop and carry out individualized care plans addressing grief, loss, adjustment, and end‑of‑life planning.
• Provide individual, family, or group counseling.
• Identify appropriate resources and coordinate care with other team members.
• Advocate on behalf of patients and families.
• Participate in interdisciplinary group (IDG) meetings, contribute to regulatory reports, and collaborate on patient care plans.
• Maintain relationships with community health care providers, agencies, and faith communities.

• Master’s degree in social work.
• LCSW or LCSWA (or in the process of obtaining).
• At least one year of experience in a mental health or medical setting.
• Valid state‑issued driver’s license; own transportation; liability insurance at agency limits.
