Required: Bachelor’s or Master’s degree in social work and two years experience in social work in a mental health or medical setting.

Authora Care Collective is currently seeking a Social Worker Home Care supporting our Greensboro service area. This is a full-time position;
Monday - Friday 8:00am - 5:00pm.

Responsible for providing intervention and assistance to the Social Work Department which is consistent with basic generalist social work practice. This includes: resource assessment and planning, intervention, evaluation, case management, information and referral, supportive presence/supportive counseling, case management, advocacy, community resource and client. Also included is long-term care planning to consist of LTC placement, completion of appropriate documentation i.e. FL-2s and obtaining PASRRs.

Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

Education/

Qualifications:

• Bachelor’s or Master’s degree in social work required.
• Two years experience in social work in a mental health or medical setting is required.
• Ability to concentrate with large volumes of work, handle pressure of deadlines, good judgment, ability to follow procedures, ability to work independently and with frequent interruptions.
• Communicate effectively with persons having highly diverse backgrounds and educational levels.
• Work effectively as a member of the interdisciplinary team.
• Other:
  Valid state-issued driver’s license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
