This role focuses on providing hands-on care in senior living settings while improving patient outcomes and helping to avoid unnecessary hospitalizations.
Contract-to-Hire, 1326 weeks
Travel within Guilford County, NC
23 patient assessments per day
Provide direct care in senior living facilities
Conduct clinical assessments to address gaps in care (STARS, HEDIS, COHPS)
Collaborate with RNs, PCPs, and facility staff
Full-time
Monday Friday, 8:00 AM 5:00 PM
Current NCCPA certification
Active North Carolina PA license
Minimum 1 year of experience as a Physician Assistant (no new grads)
Experience in senior living settings such as LTC, ALF, or SNF
