You’ll advocate for referred patients in the pre‑admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals through fostering relationships within medical facilities, and with clinical expertise and problem‑solving skills you’ll coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post‑ICU care.

• Work with physicians, discharge planners, and patients’ families to plan the best continuum of care after an ICU or post‑surgical stay.
• Perform clinical assessments of referred patients, then counsel patients’ families to assist them in making the best care decisions for their loved one.
• Partner with hospital case managers and physicians to create a smooth pre‑admission and transfer process by removing payer‑related and operational barriers for patients getting into our highly effective level of care.

• Registered Nurse (RN) or Respiratory Therapist (RT) license.
• Ability to complete clinical assessments of medically complex patients.
• Experience in patient advocacy.

• Superior communication and relationship‑building skills.
• Prior healthcare marketing or business development experience.
• Experience with payer knowledge and operational barriers.

• Full‑time:
  Monday‑Friday 8‑5 PM.
