The Primary Care Physician (MD/DO) is a key member of our interdisciplinary care team. In this role, you will evaluate patients acute and chronic conditions, manage medications, mental health, socio-emotional needs, and provide empathetic patient education. The PCP will work closely with fellow clinical teammates to provide comprehensive primary care. While direct patient care is the primary focus, PCPs also assist with development, implementation, and achievement of key organizational objectives and metrics.
Functioning as a value-based care model, our team proactively and strategically manages our population s health in accordance with IHI s Quintuple Aim, adopts best practices in coding compliance and achieves our HEDIS quality metrics. We are also encouraging Residents and Fellows at all levels to apply.
- Licensed MD/DO with board certification in Family Medicine (ABFM) or Internal Medicine (ABIM)
- Experience managing complex diseases longitudinally, including diabetes, cardiovascular disease, menopause, thyroid disease, osteoporosis, weight management, etc.
- Understanding or willingness to learn value-based care (VBC) models, including managing cost and quality of care
- Experience managing overall patient care and devising individualized care plans
- Experience educating and counseling patients on health maintenance and disease prevention
- Experience with geriatric medicine, particularly women
- Experience with polypharmacy management and appropriate medication reduction
- Experience with physical and gynecological/well-woman examinations in the elderly population
- Experience working in adaptive clinical situations
- Strong problem-solving and critical thinking skills
- Up-to-date knowledge of medical advancements and technologies
- Alignment to Herself Health mission and dedication to providing high-quality patient care
- Experience performing clinical procedures within scope of practice (skin biopsies, joint injections, endometrial biopsies)
- Value-based Care experience and/or VBC training
- Previous experience in a start-up or new clinic environment
- Strong technical skills with EMR (Athena preferred) and Microsoft Suite
- Experience mentoring, precepting and/or supervising other provider staff
- Eligibility to obtain or holder of a DEA License and an NPI number
- Regular maintenance of board certification and CME requirements
- Passage of the pre-employment background check
- Eligibility and completion of registration/credentialing with Medicare and private payers
- Proof of US Citizenship or eligibility to work in the US with a permanent resident card
- Employees in direct patient care are expected to be fully vaccinated for all communicable diseases including Hepatitis B, MMR, influenza and COVID in accordance with CDC guidelines.
