Responsibilities:
Leading Patient-Centered Care
  • Accurate Documentation:
    Maintaining accurate and thorough daily documentation of patient care, observations, and responses to treatments, ensuring comprehensive, legible, and up-to-date patient records.
  • Medication Management:
    Precisely transcribing physicians' orders onto Medication Administration Records (MARs) and preparing/safely administering medications as prescribed, adhering to all established protocols and best practices.
  • Interdisciplinary Communication:
    Promptly notifying the pharmacy of new orders and changes and proactively communicating critical patient information to the attending physician and interdisciplinary team.
  • Care Planning:
    Collaborating effectively and proactively with the entire team (CNAs, LPNs, Therapists) to develop, implement, and continuously evaluate comprehensive, individualized care plans that meet each patient's unique physical, emotional, and social needs.
  • Clinical Monitoring:
    Monitoring patient conditions closely (including vital signs, wound care, and pain levels), identifying subtle changes or concerns, and implementing appropriate, timely interventions in accordance with facility policy.
  • Patient Advocacy:
    Educating patients and their families on health conditions, medications, and self-care techniques to promote understanding, compliance, and independence upon discharge.
  • Supervision:
    Providing clinical direction and supervision to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) to ensure the highest standards of care are consistently met.
  • Performing all other duties as assigned by the Director of Nursing to ensure the smooth, efficient, and compassionate operation of patient care services.

Qualifications
  • A current and valid Registered Nurse (RN) License in Wisconsin.
  • A minimum of one year of experience in a long-term care or skilled nursing facility is strongly preferred.
  • A strong commitment to patient-centered care and a compassionate, empathetic approach.
  • Excellent communication, critical thinking, and interpersonal skills, with the ability to lead and work effectively within a team.
  • The ability to prioritize tasks, manage time effectively, and make sound clinical judgments in a dynamic healthcare environment.
