Core Clinical Responsibilities

As an LPN and clinical leader on our team, you will drive resident outcomes through:

  • Advanced Medication Management:
    Execute precise medication administration following the "five rights," ensuring 100% accuracy and adherence to individualized physician protocols.

  • Clinical Treatments:
    Perform complex wound care, respiratory treatments, and other physician-ordered procedures with technical proficiency and empathy.

  • Acute Observation:
    Serve as the "eyes and ears" of the clinical team, identifying subtle shifts in resident status and providing detailed, real-time reports to Charge Nurses or RNs to prevent hospital readmissions.

  • Interdisciplinary Synergy:
    Partner with RNs, therapists, and dieticians to refine holistic care plans, ensuring our residents receive comprehensive, 360-degree support.

  • Regulatory Stewardship:
    Maintain gold-standard compliance with state and federal healthcare mandates, upholding the integrity of our facility’s reputation.

  • Precision Documentation:
    Utilize Electronic Health Records (EHR) to maintain flawless, real-time clinical narratives and accurately transcribe complex physician orders.

Qualifications & Experience
  • Licensure:
    Active, unrestricted Wisconsin LPN License (in good standing).

  • Collaborative Mindset: A proven ability to lead CNA teams and collaborate effectively within a multidisciplinary clinical setting.

  • Technical Proficiency:
    Comfortable navigating EHR platforms and basic office software for streamlined communication.

  • Background:
    Experience in Skilled Nursing (SNF) or Long-Term Care is preferred.
