MDS Coordinator

Overview

Regal Care at Greenfield is a skilled nursing and rehabilitation center committed to delivering high-quality, resident-centered care and strong clinical outcomes. We’re currently looking for an experienced, detail-focused MDS Coordinator to join our team for a part-time, 20-hour per week position. This is a great opportunity for someone seeking flexibility while staying closely involved in clinical coordination and compliance.

• Complete and manage accurate, timely MDS assessments in compliance with state and federal regulations
• Oversee the RAI process and support interdisciplinary care planning
• Ensure documentation supports quality outcomes and reimbursement
• Collaborate with nursing, therapy, and administrative teams
• Assist with audits, surveys, and quality improvement initiatives

• Active RN or LPN license in good standing
• Prior MDS experience in a long-term care setting preferred
• Strong knowledge of Medicare/Medicaid guidelines and RAI requirements
• Excellent organizational skills and attention to detail
• Ability to work independently and meet deadlines
