Hospice RN | Full Time on Call | Mon - Thur.

The Hospice RN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands‑on care, on‑going clinical management and education regarding hospice care.

As a Hospice RN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.

This is an On‑call position

Monday - Thursday 4pm - 8am

Covering the Stark and Summit County areas

This is a salary position.

Additional responsibilities of our Hospice RN include:

• Continually evaluating the patient's needs
• Initiating, coordinating and revising the treatment plan
• Rendering nursing care and performing treatments, as ordered by the physician
• Participating in the Quality Assurance/Quality Improvement process
• Preparing accurate, orderly and timely reports and documentation
• Adhering to all practice standards as they apply to patient care

• Minimum 2 years of nursing experience must include hospice experience.
• Caring, patient and flexible teacher with excellent communication and time management abilities
• Possess independent problem solving abilities, excellent clinical assessment skills
• Ability to lift/transfer patients to and from wheelchairs, if necessary
• Associate's of Science degree in Nursing
• Valid driver's license and one year clean driving record
• Excellent writing, communication and charting skills
