RN Case Manager / Nursing Supervisor

Job Title:

RN Case Manager / Nursing Supervisor

Service Area: 4+ county radius in North East Ohio. Includes Summit, Medina, Cuyahoga, Tuscarawas, and surrounding counties primarily aligned along I-77. Typical travel distances require a 30-90 minute daily radius, with occasional travel up to 2 hours to cover all county cases.

Schedule Type:
Flexible and customizable. Typical hours are Monday-Friday during the day (8am-5pm).

Employment Type:

Part-time long-term employment. No on-call requirements or overnight shifts. Weekend, overtime, respite & PRN services are offered when available. Part-time RNCMs receive hourly compensation paid weekly on Fridays.

Travel Requirements:
Willingness to travel throughout all stated counties in this service area to care for clients is required.

Qualifications:

• Graduate of an accredited School of Nursing.
• Minimum of 1 year experience as a Registered Nurse, with an active RN License in the State of Ohio.
• CPR and First Aid Certifications required.
• 1 year experience in case management and home health required; 2+ years of home health experience highly preferred.
• Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
• Strong background in nursing, patient care, and confidentiality.
• Strong attendance and dependability track record with previous employers.
• Current Driver's License and reliable transportation.
• Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented in care + documentation, accountable, friendly and accommodating, respectful, and positive.
• Familiarity with HIPAA regulations and medical terminology.

Responsibilities:

• Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
• Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
• Document, communicate, and coordinate patient status/change in condition to care team.
• Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
• Minimum of one (1) year of nursing supervisory experience. Home care experience preferred.
• Travel throughout all stated counties in this service area to care for clients is required.

Compensation:

• Part-time Hourly Wage: $35 - $40/hour
