Job SummaryThe Family Medicine/Urgent Care Advanced Practice Provider (APP) at Prevea is a Nurse Practitioner (NP) or Physician Assistant (PA) prepared to deliver comprehensive health care services to patients, families, and groups. This role emphasizes health promotion, disease prevention, and the management of acute and chronic conditions across all ages.
As a Float APP, this position provides flexible support across Primary Care and Urgent Care locations, delivering both primary and prompt care for illnesses and injuries that require timely attention but do not necessitate emergency department visits. The APP functions with a high degree of autonomy while collaborating and coordinating with physicians and other health professionals to ensure high-quality, patient-centered care.
Responsibilities also include participation in clinical practice, education, consultation, research, and program development initiatives within the Prevea Health system.
- Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries. Including performance of follow up exams if required.
- Collaborates with patients to review their health background and determine physiological, emotional, and environmental needs, using this information to advise patients and their families on future health plans and illness prevention.
- Provides case management services to assist clients and families with the arrangement for, or refer patients to needed services.
- Provides services, which may include the ordering of appropriate diagnostic and laboratory tests, the prescription of pharmacological agents, and other non-pharmacological treatment modalities. Also at times conducting
- Maintains and reviews patient records, charts, and other pertinent information, documenting all details in the electronic medical record.
- Conducts telephone calls and electronic communications back to patients requiring follow-up care or provide test results. Also provides health counseling and education to patients and their families about their healthcare and any follow-up instructions.
- As needed per site/department needs this position is responsible for checking in patients, collecting payments, scheduling appointments, rooming patients, performing lab draws, and conducting point-of-care testing to ensure efficient patient flow and quality care.
- Collaborate with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients as needed.
- Master's Degree Graduate of accredited master's level Family Nurse Practitioner or accredited Physician Assistant program Required
- 1-3 years work experience in a healthcare setting Preferred
- Knowledge of the information and techniques required to diagnose and treat human injuries and diseases, including understanding symptoms, treatment options, drug properties and interactions, and preventive healthcare measures.
- Demonstrate organizational skills that allow timely patient appointments and appropriate required follow up.
