This position will work 36 hours per week as a Registered Nurse.

• Manages patient/family health concerns through telephone triage and consultation.
• Actively seeks information to understand patient circumstances, problems, expectations, and needs.
• Facilitates caller s access to appropriate level of care by assessing needs and offering clinical options using medical judgement within scope of licensure.
• May also refer to health care providers, services, and community resources.
• Collaborates with other healthcare providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.
• Conducts telephone calls back to patients requiring follow-up care or to provide test results.
• Provides health counseling and education to patients and their families about their healthcare and any follow-up instructions.
• Provides direct patient care which includes, but is not limited to rooming of patients, obtaining medical information, performing comprehensive initial patient assessment, and maintaining patient charts/records.
• Assists the Physician or Advanced Practice Provider with procedures, notes their instructions/ordres and makes necessary arrangements to ensure they are carried out.
• Direct patient care may also include medication administration, and communicating information with patients, Physician, Advanced Practice Providers and staff to ensure quality and continuity of care.

• Registered Nurse with licensure in Wisconsin; scope of practice as defined by the Wisconsin Board of Nursing and Prevea s policy.
• Experience in triage, patient communication, and coordination of care is implied by responsibilities described.
