Job SummaryThe Registered Nurse at Prevea plays a crucial role as an integral member of our clinic team, delivering patient-centered care. Responsible for triaging incoming calls from patients and families, assessing their needs, giving appropriate advice, and facilitating appointments with physicians and advanced provider clinicians. The Registered Nurse position will also provide hands‑on direct patient care as needed. The Registered Nurse is accountable for the delivery of exceptional and safe care within the scope of practice as defined by the Wisconsin Board of Nursing and Prevea's policy.
Through collaborative practice with members of the Prevea team, they are responsible for patient outcomes that meet the high quality of care provided by Prevea.
- Manages patient/family health concerns through telephone triage and consultation. Actively seeks information to understand patient circumstances, problems, expectations, and needs.
- Facilitates caller's access to appropriate level of care by assessing needs and offering clinical options using medical judgement within scope of licensure. May also refer to health care providers, services, and community resources.
- Collaborate with other healthcare providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.
- Conducts telephone calls back to patients requiring follow‑up care or provide test results. Also provides health counseling and education to patients and their families about their healthcare and any follow‑up instructions.
- Provides direct patient care which includes, but it is not limited to the rooming of patients, obtaining medical information, performing comprehensive initial patient assessment, and maintaining patient charts/records. Assists the Physician or Advanced Practice Provider with procedures, notes their instructions/orders and makes necessary arrangements to ensure they are carried out.
- Direct patient care may also include medication administration, and communicating information with patients, Physician, Advanced Practice Providers and staff to ensure quality and continuity of care.
- Graduation from an accredited and approved Nursing program – Required
- Bachelor's Degree in Nursing – Preferred
- Skilled in establishing and maintaining effective working relationships with patients, medical staff, and the public
- Excellent interpersonal skills and behaviors
- Knowledge of common safety hazards and precautions to establish a safe work environment
- Competent in patient education/teaching
- Ability to assist providers with exams, tests & treatments
- Strong patient assessment skills in person and on the phone in a high‑volume, fast‑paced environment
- Ability to administer immunizations and medications
- Current and valid Registered Nurse license with the state of WI – Upon Hire Required
- CPR/BLS Certification – Basic Life Support for Healthcare Providers certification from an approved program. Upon Hire Required
