Job Summary The Registered Nurse at Prevea plays a crucial role as an integral member of our clinic team, delivering patient-centered care. Responsible for triaging incoming calls from patients and families, assessing their needs, giving appropriate advice, and facilitating appointments with physicians and advanced provider clinicians. The position will work 36 hours per week and will also provide hands‑on direct patient care as needed.

The Registered Nurse is accountable for the delivery of exceptional and safe care within scope of practice as defined by the Wisconsin Board of Nursing and Prevea's policy. Through collaborative practice with members of the Prevea team, they are responsible for patient outcomes that meet the high quality of care provided by Prevea.

• Manages patient/family health concerns through telephone triage and consultation.
• Actively seeks information to understand patient circumstances, problems, expectations, and needs.
• Facilitates caller’s access to appropriate level of care by assessing needs and offering clinical options using medical judgement within scope of licensure.
• May also refer to health care providers, services, and community resources.
• Collaborate with other healthcare providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.
• Conducts telephone calls back to patients requiring follow‑up care or to provide test results.
• Also provides health counseling and education to patients and their families about healthcare and follow‑up instructions.
• Provides direct patient care which includes, but is not limited to, rooming of patients, obtaining medical information, performing a comprehensive initial assessment, and maintaining charts/records.
• Assists the Physician or Advanced Practice Provider with procedures, notes instructions/orders, and makes necessary arrangements to ensure they are carried out.
• Direct patient care may also include medication administration, and communicating information with patients, physicians, advanced practice providers, and staff to ensure quality and continuity of care.
