ScheduleFull‑time, 36 hours per week. Monday and Tuesday 8:00 AM–5:00 PM, Wednesday 10:00 AM–8:00 PM, Thursday off, Friday 7:00 AM–5:00 PM. On‑call and holiday responsibilities: one full weekend every other month or one weekend shift per month;
Friday 5 p.m.–9 p.m., Saturday and Sunday 8 a.m.–5 p.m. or 11 a.m.–8 p.m.; one holiday per year, 8 a.m.–5 p.m. or 11 a.m.–8 p.m.
Proficiency in Microsoft Office (Word, Excel, Outlook). Highly motivated, organized, and detail‑oriented with analytical skills. Effective verbal and written communication. Confidentiality, discretion, sound judgment, and compliance with all applicable laws, regulations, and Unity policies. Keep up to date on policies, practices, and regulations. Prioritize and handle multiple tasks under deadline pressures. Perform other job‑related duties as assigned. Adjust to frequently changing demands.
WhatYou’ll Do
- Complete initial psychosocial assessments and establish individualized plans of care.
- Provide emotional support, education, crisis counseling, and advocacy.
- Communicate regularly with patients, families, caregivers, and facility partners.
- Assess bereavement risk and support grief services as needed.
- Coordinate community resources and referrals to ensure safe, appropriate care in the home.
- Assist families with funeral planning upon request.
- Participate in IDT meetings, plan‑of‑care reviews, and team collaboration.
- Work with Volunteer Services to align volunteers and coordinate veteran pinning ceremonies.
- Support process improvements and use Unity’s specialty programs (Gifted Wishes, Holistic Harmonies, Respecting Valor).
- Maintain accurate, timely documentation and adhere to the Social Work Code of Ethics.
- Participate in the 24/7 coverage rotation and maintain dependable attendance.

- Bachelor’s in Social Work (BSW) from an accredited institution or bachelor’s degree in psychology, sociology, or related fields plus one year in a healthcare setting. Master’s of Social Work preferred.
- Certified or licensed as a social worker (CSW, CAPSW, CISW, or LCSW) by the State of Wisconsin required.
- Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
