Education, Training & Experience Required

• Board certified/board eligible Pain Medicine or Physical Medicine & Rehabilitation
• Fellowship trained preferred, but not required
• EMG training/experience preferred, but not required
• Licensed in the State of Wisconsin or willing to obtain license
