Job SummaryA qualified Licensed Practical Nurse (LPN or LVN) at First Light Healthcare Solutions, performs services as directed by the Registered Nurse.
Licensed Practical/Vocational Nurse (LPN/LVN) responsibilities include:
- Provides skilled nursing services under the direction of a Registered Nurse.
- Follows the plan of care and reports necessary revisions to the Registered Nurse.
- Provides services in accordance with the plan of care.
- Provides documentation of client assessments, plan of care, supervisory visits, caregiver and client education and training.
- Informs client and/or client representative of changes in the condition and needs of the client.
- Understands and adheres to established policies and procedures.
- Teaches and counsels the client and family/significant others regarding their service needs and other related problems of the client at home when applicable.
- Maintains accurate and complete records
- Attends staff meetings, client care conferences, and in-services as scheduled.
- Supervises and teaches other personnel, when applicable.
- All other duties as assigned.
- Graduate of a state-approved school of nursing and currently licensed in the state in which practicing
- Maintains CPR Certification
- Minimum of one (1) year experience in Home Care, preferred.
- Acceptance of the philosophy and goals of the Agency.
- Ability to exercise initiative and independent judgment.
