Job Summary

A qualified Licensed Practical Nurse (LPN or LVN) at First Light Healthcare Solutions, performs services as directed by the Registered Nurse.

Licensed Practical/Vocational Nurse (LPN/LVN) responsibilities include:

  • Provides skilled nursing services under the direction of a Registered Nurse.
  • Follows the plan of care and reports necessary revisions to the Registered Nurse.
  • Provides services in accordance with the plan of care.
  • Provides documentation of client assessments, plan of care, supervisory visits, caregiver and client education and training.
  • Informs client and/or client representative of changes in the condition and needs of the client.
  • Understands and adheres to established policies and procedures.
  • Teaches and counsels the client and family/significant others regarding their service needs and other related problems of the client at home when applicable.
  • Maintains accurate and complete records
  • Attends staff meetings, client care conferences, and in-services as scheduled.
  • Supervises and teaches other personnel, when applicable.
  • All other duties as assigned.
The ideal candidate will have:
  • Graduate of a state-approved school of nursing and currently licensed in the state in which practicing
  • Maintains CPR Certification
  • Minimum of one (1) year experience in Home Care, preferred.
  • Acceptance of the philosophy and goals of the Agency.
  • Ability to exercise initiative and independent judgment.
