PositionSummary
The Child and Youth Systems of Care Manager will oversee the development, implementation, and management of a comprehensive system of care for children and youth. This role is responsible for ensuring that NHPs CO‑SOC and Enhanced Standardized Assessment (ESA) processes are integrated, compliant, and operating with high fidelity. The Manager provides direct consultation, guidance, and escalation support internally and externally; drives cross-system planning for high-acuity youth;
ensures timely completion of contracted care-coordination activities; and establishes strong partnerships with county departments, Case Management Agencies (CMAs), Behavioral Health Administrative Service Organizations (BHASOs), Primary Care Medical Providers (PCMPs), Community Safety Net Providers/Essential Safety Net Providers (CSNPs/ESNPs), and residential/facility partners to ensure children and youth receive coordinated, family-centered, and culturally responsive care.
- Design, implement, and evaluate systems of care programs and services that support children and youth with emotional, behavioral, and developmental challenges.
- Oversee and provide subject-matter expertise for care-coordination activities for children and youth, including assessment completion, care plan development, interdisciplinary case planning, and ongoing monthly engagement.
- Lead and facilitate Creative Solutions Meetings for high-acuity or multi-system youth, ensuring coordinated action planning, clear documentation, and follow-through across RAE and provider partners. Carry a limited complex caseload when clinically or operationally necessary to model best-practice CO‑SOC care coordination and support system navigation. Provide case consultation and quality oversight to care coordination staff, reviewing care plans, documentation, and engagement strategies to ensure fidelity to NHP standards and ACC 3.0 requirements.
- Ensure fidelity to ESA completion workflows, coach care coordinators on appropriate administration and documentation, and troubleshoot cross-agency coordination challenges to support timely residential placement evaluations or other service determinations.
- Provide high-level expertise and troubleshooting on transitions of care for children and youth, including psychiatric hospitalizations, residential treatment discharges, detention releases, and emergency/crisis placement disruptions.
- Establish and maintain strong partnerships with CMAs, BHASOs, PCMPs, CSNPs/ESNPs, counties, school districts, and community organizations to promote shared-care planning, warm handoffs, and coordinated case management across systems.
- Lead recruitment and retention efforts for CO‑SOC specialty providers, including EFFT, EMST, and EHFW clinicians and agencies.
