Salary: £20,492 - £25,401 per annum

Young Adults Team (YAT) All Age Disability – Permanent

The role:

The All Age Disability Area Teams are statutory social work teams, combining the functions of the Children with Disabilities Team and the Young Adults Team to ensure a seamless service through childhood and into adulthood. We work with children and young people with a disability that has a significant impact on their daily lives, by identifying their need and desired outcomes and developing plans with them to ensure that they reach their potential.

We work in partnership with SEND Services to ensure that social care plans and the EHC plan is aligned and focused on good outcomes for the child/ young person or young adult.
