Admission Registered Nurse, Hospice

This role involves determining a patient's eligibility for hospice following a physician referral. Responsibilities include screening for insurance benefits in collaboration with the hospice account representative, working directly with the case manager and social worker after patient admission, and conducting initial assessments using age-specific physical, psychological, and social evaluations to enhance comfort and health of terminally ill patients and their families.

The role also participates in the performance improvement program.

The candidate must demonstrate the ability to work under pressure to meet deadlines, maintain accuracy, and adapt to changing situations. Effective communication with a variety of people, stress management, and conflict resolution skills are essential.

All assigned duties must be performed in accordance with organizational policies, procedures, and relevant licensing requirements.

• Current state registered nurse license required.
• Graduate of an accredited nursing school.
• Experience as an RN, ideally with one or more of the following within the last two years:
• One-year acute care or long-term care experience;
• One-year home health care experience;
• One-year community health or public health experience;
• One-year hospice experience.
