- Coordinate meetings with case managers, clinicians, community-based providers, and/or support workers.
- Ask questions to ensure clarity about changes in care, resources, and treatment.
- Recognize the demands of caring for individuals with challenging mental health issues and organize respite when needed.
- Double-check all appointment times and locations.
- Request information and appointment details in writing, such as copies of forms, letters, or emails.
- Gather information about the consumer’s history and plan for the next stage of care.
- Identify organizations or agencies involved in care and treatment.
- Maintain a caseload established with the Service Director and work closely with programs in assigned service areas.
- Assess and evaluate adults to determine the appropriateness and need for case management services.
- Assist in the person’s entry into service and monitor needed services throughout the service system.
- Provide coordination and comprehensive development of an Individual Treatment Plan (ITP) and monitor that plan as required by established rules.
- Follow State of Montana and A.W.A.R.E., Inc. rules, policies, procedures, and guidelines as they apply to this service.
- Ensure that any person serviced by Adult Case Management is screened for and assisted in enrollment in the SDMI Program.
- Demonstrate a consistent willingness and ability to productively utilize work time to meet the needs of the individuals being served, meeting specific fiscal goals such as billing and caseload maintenance on a monthly basis.
- Record notes, billing units, assessments, referrals, and other data entry assignments via AWARE’s Electronic Healthcare Record System.
- Ability, under AWARE’s Unconditional Care Philosophy, to develop and implement individual treatment plans with knowledge of behavioral, clinical, and mental health interventions, skill‑building techniques, and understanding of clients’ emotional and behavioral needs.
- Strong interpersonal oral and written communication skills to work as a team member, observe accurately, and report findings.
- Bachelor’s degree in a Human Services field and a minimum of one year of direct work experience, or an equivalent combination of education and experience.
- Valid Montana Driver’s License and a clean driving record; a satisfactory criminal and protective services background check.
AWARE adult case managers earn $20.87
–$22.19 per hour.
