Hospice RN Case Manager

Overview

We are hiring full-time, part-time, and per diem RN Case Managers to join the Hospice of Western & Central Massachusetts team in Berkshire County. Up to an $8,000 sign-on bonus. This is an on-the-road position that requires travel and a reliable, insured vehicle. Weekly pay.

• Supervise hospice aides.
• Complete an initial, comprehensive, and ongoing comprehensive assessment of the patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Provide professional nursing care by utilizing all elements of the nursing process.
• Assess and evaluate the patient’s status by writing and initiating a plan of care, regularly re-evaluating the patient and family/caregiver needs, and participating in revising the plan of care as necessary.
• Initiate the plan of care and make necessary revisions as patient status and needs change.
• Use health assessment data to determine nursing diagnosis.
• Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions; include the patient and family in the planning process.
• Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician in the plan of care.
• Counsel the patient and family in meeting nursing and related needs.
• Provide health care instructions to the patient as appropriate per assessment and plan.
• Assist the patient with activities of daily living and facilitate the patient’s efforts toward self-sufficiency and optional comfort care.
• Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for the assigned caseload.

• Graduate of an accredited school of nursing; bachelor’s degree preferred.
• Must possess an active and unencumbered RN license in Massachusetts.
• One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care, preferred.
• CPR certification.
