Role Summary

As a Full‑Time Registered Nurse Case Manager, you will create, implement, and execute comprehensive care plans to promote wellness and prevent re‑hospitalization for patients in their homes.

Territory

Grass Valley, Colfax & Surrounding Cities.

Responsibilities
  • Perform a full assessment of the patient and family during the initial visit.
  • Use assessment data to develop an advanced plan of care, including rehabilitation.
  • Provide education and guidance to patients and caregivers on wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, CHF management, and medication.
  • Case‑manage the patient care episode with an interdisciplinary clinical team.
  • Prepare clinical notes, orders, and charting in a compliant timeframe.
  • Collaborate with physicians and agency staff to deliver optimal care.
  • Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively per established protocols.
Qualifications
  • Current, unrestricted California RN license.
  • Current BLS/CPR certification.
  • Valid California driver’s license.
  • High level of clinical knowledge in home health care (highly preferred).
  • Experience with high‑acuity patients (preferred).
  • Experience working with an interdisciplinary team (desired).
  • Case‑management fundamentals (desired).
  • Previous experience with IV medication infusions (highly preferred).
  • Proficiencies in initiating intravenous therapy (plus).
  • Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.
  • Must be willing to work either Saturday or Sunday.
