As a Full‑Time Registered Nurse Case Manager, you will create, implement, and execute comprehensive care plans to promote wellness and prevent re‑hospitalization for patients in their homes.
TerritoryGrass Valley, Colfax & Surrounding Cities.
Responsibilities- Perform a full assessment of the patient and family during the initial visit.
- Use assessment data to develop an advanced plan of care, including rehabilitation.
- Provide education and guidance to patients and caregivers on wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, CHF management, and medication.
- Case‑manage the patient care episode with an interdisciplinary clinical team.
- Prepare clinical notes, orders, and charting in a compliant timeframe.
- Collaborate with physicians and agency staff to deliver optimal care.
- Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively per established protocols.
- Current, unrestricted California RN license.
- Current BLS/CPR certification.
- Valid California driver’s license.
- High level of clinical knowledge in home health care (highly preferred).
- Experience with high‑acuity patients (preferred).
- Experience working with an interdisciplinary team (desired).
- Case‑management fundamentals (desired).
- Previous experience with IV medication infusions (highly preferred).
- Proficiencies in initiating intravenous therapy (plus).
- Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.
- Must be willing to work either Saturday or Sunday.
