You will be a registered nurse,holding a degree or equivalent experience in adult nursing and will be responsible for the delivery of the full practice nursing service to our patient population.
This will include chronic disease management, immunisation and vaccination,treatment room services and health prevention and screening programmes. You willbe joining our existing team to provide innovative care as well as working withour primary care network to deliver services at scale.
You will be a friendly and personable team player, with good communication skills and have a willingness to embrace change and manage your workload effectively. Our team are committed to providing high quality care and we encourage everyone to share knowledge andlearn together.
We support personal development and will be keen for you to develop your skill set as well ascompleting the non-medical prescribing qualification.
Main duties of the jobThe role will focus on all areas of practice nursing, including chronic disease management, treatment room services, health screening and immunisation and vaccination.
About usYou will be joining an established practice team with 3 GPs, 1 GP Registrar, a Practice Nurse and Phlebotomist. We have a practice list size of 4000 patients, and we deliver care from two village locations. We have a rating of Good with CQC, and we consistently achieve high QOF aspirations.

Job responsibilitiesAssist in and perform routine tasks related to patient care as directed by clinical colleagues.
- Chronic Disease Management
- Treatment Room Services
- Wound care
- Undertake cytology
- Travel Health and Vaccination
- Childhood Immunisation
- Ordering of immunisations and vaccinations and overseeing all associated checks pertaining to the cold chain
- Venepuncture
- Assisting GPs with minor surgery
- Requesting pathology tests, for example urine culture, swabs
- Active lead in the monitoring/call/recall of patients for QOF to ensure targets are met.
- Maintain and update clinical protocols and liaise with the Clinical Lead
- Infection Prevention and Control
- Overseeing Emergency Equipment checks
- Stock control, rotation and ordering
- Maintain all clinical equipment
- Attend and participate in Practice meetings.
- Participate in extended hours clinics
- Undertake home visits
- Carry out any other delegated duties appropriate to the post

Health & Safety:
The post-holder will implement and lead on a full range of promotion and management their own and others health and safety and infection control as defined in the practice Health & Safety Policy, the practice Health & Safety Manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):
- Using personal security systems within the workplace according to Practice guidelines
- Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines.
- Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
- Management and maintenance of Personal Protective Equipment (PPE)
- Responsible for hand hygiene across the practice
- To take a lead on Infection Control within the practice and take ownership of all associated policy. Undertaking audit and responding to any areas of concern.
- Identifying any risks involved in clinical activity and service delivery and managing change where appropriate.
- Ensure that knowledge and skills are kept up to date and initiate and manage the training of others in relation to infection control.
- Monitoring practice facilities and equipment.
- Safe management of sharps procedures including training, use, storage and disposal
- Maintaining a safe and clean working environment
- Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised
