Health Navigator

Chestnut Family Health Center is seeking to hire a full-time Health Navigator in our Belleville, IL facility. This role supports patients in navigating the complexities of behavioral health care and is well-suited for someone with prior experience in substance use or mental health disorders and background working with patients in health care settings. The position offers opportunities to develop skills in social work, psychology, counseling, and related behavioral health fields.

• Help facilitate patient access to quality primary and behavioral health care.
• Provide clinician support using social work and psychology theories and models to reduce symptoms related to DSM-5 diagnoses.
• Provide educational resources on care and processes with patients  social service encounters.
• Screen clients for issues related to Social Determinants of Health and make service referrals.
• Facilitate communication among patient, family members, and providers.
• Manage follow-up visits.
• Ensure continuation of care.
• Link patients to financial resources.
• Arrange for interpretive language services as needed.
• Promote the evidence-based recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness supported through social work and counseling practices.

Minimum of a high school diploma with 10 years’ experience in the human services field, health care, or advocacy; OR a Bachelor’s degree with 5 years’ experience as stated above; a Master’s degree is preferred. Valid driver’s license, private automobile insurance and be insurable. Requires basic computer skills and experience with MS Word, MS Excel, Outlook, an electronic health record (EHR), and general knowledge of standard office procedures and use of office equipment (filing, copier, fax, and scanning).

The anticipated starting pay for new hires for this position is between $44,400 - $50,575 annually. There are several factors considered in determining base pay, including job-related qualifications, skills, education, experience, local market conditions, and internal equity.
