Your Impact:

As a Home Health RN, you'll be the heart of your patients' care:

* Lead your caseload with autonomy-assess, plan, and deliver care tailored to each patient

* Provide compassionate guidance and support to patients and families

* Coordinate care with doctors, therapists, and caregivers to keep everyone on the same page

* Adapt plans as patients' needs evolve, ensuring the best outcomes

* Maintain HIPAA-compliant documentation while being the trusted voice patients rely on

What We're Looking For:

* Active RN license with 1+ years of clinical experience

* Comfort traveling within the territory (about 50% travel)

* Reliable transportation, valid driver's license, and auto insurance

* Home Health or Hospice experience is a plus

* Associate or Bachelor's in Nursing preferred

* Experience with Home Care Home Base is a bonus
