As a Home Health RN, you’ll be the heart of your patients’ care:

• Lead your caseload with autonomy—assess, plan, and deliver care tailored to each patient

• Provide compassionate guidance and support to patients and families

• Coordinate care with doctors, therapists, and caregivers to keep everyone on the same page

• Adapt plans as patients’ needs evolve, ensuring the best outcomes

• Maintain HIPAA-compliant documentation while being the trusted voice patients rely on

• Active RN license with 1+ years of clinical experience

• Comfort traveling within the territory (about 50% travel)

• Reliable transportation, valid driver’s license, and auto insurance

• Home Health or Hospice experience is a plus

• Associate or Bachelor’s in Nursing preferred

• Experience with Home Care Home Base is a bonus
