POSITION OVERVIEW:
The Elders Support Coordinator is a two-year fixed term position that supports DCN Elders by providing information, advocacy, programming, and coordinated services to enhance their social, cultural, and overall well-being. This role delivers group programming and case management services, conducts home visits and wellness checks, assists with applications and referrals, and ensures Elders are connected to community and external resources. The Coordinator works collaboratively to reduce isolation, promote cultural engagement, and ensure Elders’ needs are met in accordance with applicable policies and legislation.
KEY RESPONSIBILITIES:
Program Coordination & Community Engagement
- Plan and facilitate social, cultural, recreational, and educational activities for Elders
- Organize Elder dinners/meetings, prepare agendas, and record minutes
- Coordinate special events (e.g., Christmas, Easter, Thanksgiving, Elders Day)
- Arrange travel to conferences, workshops, and community events
- Provide transportation support and ensure proper maintenance of program vehicle
- Promote awareness of programs and community events
Case Management & Client Support
- Conduct needs assessments, home visits, wellness checks, and follow-ups
- Assist with applications (e.g., seniors’ benefits, income tax, will kits, provincial programs)
- Develop client action plans and review regularly
- Determine eligibility for supports per policy guidelines
- Maintain confidential client files and documentation
- Provide referrals, advocacy, and wrap-around service coordination
Administration & Reporting
- Maintain financial records and manage Elder program budgets
- Submit monthly and required reports to the Director
- Maintain accurate client documentation and program records
- Seek additional funding opportunities and support proposal development
QUALIFICATIONS AND REQUIREMENTS:
- Post-secondary education in Human Services, Health Sciences, or related field preferred
- Experience in seniors’ services, in-home support, and group facilitation
- Valid Alberta Driver’s License (Class 4 asset), reliable vehicle, and acceptable driver’s abstract
- First Aid Level 1 (or Emergency First Aid – Community Care)
- Mental Health First Aid
- Criminal Record Check
- Intervention Record Check
- Strong communication, organizational, case management, and computer skills (MS Office)
- Ability to work independently and in a team environment
