Overview

We’re Hiring:
Foster Care Case Manager – Grand Rapids, MI!

Salary: $47,008

Make a real impact in the lives of children and families. We’re looking for a compassionate Case Manager to provide support, advocacy, and stability for youth in foster care.

Responsibilities
  • Manage and support your own caseload
  • Coordinate placements & attend court hearings
  • Work closely with children, families, and community partners
  • Provide social work and case management services for assigned cases, in accordance with contractual and regulatory requirements and accreditation standards
  • Assume primary responsibility for all cases on assigned caseload, including a range of case management activities (social casework, engagement, mentoring, and community referral)
  • Collect, assess, and document data and client progress; complete documentation, reports, statistics, and assignments within required time frames
  • Collaborate with referral sources, agencies, institutions, and individuals involved in the client’s life situation
  • Prepare for and attend all scheduled court hearings on assigned caseload
  • Take primary responsibility for intake and initial placement services for all newly referred cases
  • Coordinate placement arrangements with caregivers, including transportation of children as needed
  • Provide more intensive supportive casework to the child, birth family, and caregiver during initial placement and replacement
Job Qualifications

Education, Training, and Licensure/Certification

  • Bachelor’s degree in Social Work or related human service field
  • Licensed by the State of Michigan preferred

Experience

  • One year experience in child welfare, preferred

Additional Work Requirements

  • Participate in a 24 hour availability system to clients, foster and adoptive parents, as required
  • Unrestricted valid driver’s license with a good driving record; reliable personal transportation suitable for safely transporting persons served; proof of current automobile insurance
