Overview
As an Unaccompanied Children (UC) Transitional Foster Care (TFC) Case Manager, this position is primarily responsible for providing casework and supportive services to children in foster care, foster families, and biological families referred to refugee and immigrant services TFC program. This position must provide child-facing services on-site. This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements.

About Catholic Charities West MichiganCatholic Charities West Michigan (CCWM) is a community-based agency providing programs in Western Michigan and offering a variety of services in family preservation, behavioral health, child welfare, and community outreach.
Responsibilities- Coordinate case management services for children in care under the direction of the Lead Clinician/Lead Case Manager, including coordinating timely referrals to external care providers and maintaining consistent communication with relevant stakeholders on referral status.
- Ensure consistent care coordination communication between medical and behavioral health staff internally and externally, coordinating timely referrals to child advocate services as needed, ensuring appropriate health interpretation and translation services are available, and coordinating post-release services that include continuity of healthcare services.
- Complete initial intake assessment within 24 hours of arrival and ensure youth feel safe and are well oriented to the agency and foster home.
- Coordinate contact with youth sponsors promptly upon arrival and ensure the sponsor obtains applicable documentation such as the Family Reunification program (FRP) packet.
- Identify a sponsor by working with the consulate or equivalent resources to assign a sponsor for the youth, if unknown upon arrival.
- Apply a trauma-focused lens to complete the FRP in a timely manner to ensure youth safety and release.]
- Ensure all assigned youth receive mandatory services in care, including mental health, medical, recreational, educational, and other supportive services within program time expectations.
- Maintain accurate and current information on foster children in care, ensuring ORR and CCWM internal documentation practices and reporting requirements are met.
- Attend weekly meetings with supervisor to consult on cases, review case plans and progress, and evaluate client participation.
- Assist the licensing team to ensure foster families are well trained and equipped to support UC youth in their home.
- Schedule monthly meetings with foster parents and foster children in the foster home and address any concerns in service delivery.
- Translate written and verbal communication for program clientele and staff, if bilingual.
- Maintain effective communication with community agencies or individuals involved in service provision.
- Attend internal and external trainings and apply newly gained knowledge in providing services.
- Attend and prepare for required court hearings, if applicable.
- Transport clients as needed.
- Provide child-facing services on-site.
- Drive for agency business.

Education
- A bachelor’s degree in behavioral sciences, human services or social services fields is required.
Experience
- A minimum of 1 year of prior employment experience in child and family services or counseling is strongly encouraged.
- A minimum of 1 year of prior employment experience working with children and families from diverse cultures with sensitivity to cultural difference and norms is strongly preferred.
- Bilingual Spanish/English fluency is highly preferred.
Professional Certificates, Licenses, and Registrations
- Valid CPR and First Aid certified upon hire or within 60 days of hire is required.
- Must pass an agency-approved background check.
