Key Responsibilities :

Provide high-quality nursing care to patients in home care or private duty settings.

Conduct patient assessments and develop individualized care plans.

Administer medications, injections, and treatments as prescribed.

Monitor vital signs, symptoms, and patient progress .

Assist patients with Activities of Daily Living (ADLs) while promoting independence.

Maintain accurate clinical documentation and patient records.

Educate patients and families regarding health conditions, medications, and care plans .

Collaborate with healthcare professionals and family members to coordinate patient care.

Ensure patient safety, infection control, and comfort at all times.
Qualifications :

Active LPN or RN license .

Experience in private duty nursing, home health, or long-term care preferred.

Strong knowledge of patient care, medication administration, and clinical assessments .

Experience with wound care, tube feeding, or ventilator support is a plus.

Excellent communication and interpersonal skills .

Ability to provide compassionate, patient-centered care .
