Our organization is a growing Hospice Agency serving the Kent County area from our Troy Office. We are seeking compassionate, dynamic, energetic, organized, and efficient professionals to join our Hospice Team.

• Provide one-on-one home-based care to patients.
• Support patients and families with dignity and heart.
• Spend real time where it matters most and deliver care without hospital distractions.
• Conduct supervisory visits to observe the performance of assigned team members.
• Participate in regular interdisciplinary group meetings and contribute to the development, review, and revision of the patient/family plan of care.
• Ensure high-quality care and maintain patient/family satisfaction.
• Utilize current processes and technology to communicate information within the organization.

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to participate in field visits daily and weekly office meetings as indicated.
• On-Call responsibilities for the position (full-time on-call basis).
• Ability to adapt to patient status and needs in a dynamic home-care setting.
