Ever Home, a skilled, Medicare-certified home health agency, is seeking a compassionate and dedicated Nurse (RN) to support residents at two large Independent and Assisted Living communities in Jenison, MI. This is a unique role where you’ll serve as the primary home health nurse for the two communities, coordinating care with physicians and a team of therapists.
What You’ll Do- Conduct Initial & Ongoing OASIS Assessments
- Develop and oversee the plan of care with the physician and home health team
- Provide Skilled Nursing Interventions including wound care, symptom management and post hospitalization recovery support
- Chronic Disease Management & Teaching
- Medication Management
- Patient & Caregiver Education
- Active Michigan RN license
- Experience in home health preferred
- Strong communication and relationship-building skills
- Compassion and commitment to working with older adults
