PositionWe are seeking an Advanced Nurse Practitioner responsible for overseeing hospice patient care, implementing and evaluating the Plan of Care, and collaborating with physicians.
Responsibilities- Ensuring compliance with regulations, reviewing diagnostic results, managing medication adjustments, maintaining documentation, and serving as the primary on‑call clinician for urgent needs.
- Master’s Degree in Nursing
- Current licensure
- Active DEA license
- At least 1 year of hospice or home care experience
- Knowledge of hospice principles and geriatric medicine
- Certification in hospice and palliative care preferred
