Hospice RN Case Manager

• Conduct ongoing assessments of patients’ physical and psycho-social status
• Collaborate with the Interdisciplinary Group (IDG), patients, families, and attending physicians to develop care plans
• Provide education and interventions to manage pain and other symptoms effectively
• Ensure availability of necessary equipment, supplies, and medications
• Revise care plans as patients’ needs change
• Document patient and family services accurately and promptly
• Participate regularly in IDG meetings

• Active, unrestricted RN/LPN license in the State of Michigan
• At least one (1) year of relevant experience as an RN or LPN (required)
• Preferred:
  One (1) year of management experience
• Preferred:
  One (1) year of home health experience
• Current CPR Certification
• Preferred:
  Community health and a broad range of Medical-Surgical experience
• Reliable transportation with a valid state driver's license and current car insurance
