• Serve as a vital contact for referral sources, outside agencies, health professionals, and field staff.
• Coordinate incoming calls, providing essential information to patients and referral sources.
• Communicate with patients and families to arrange care/services initiation.
• Conduct intake screenings, documenting and analyzing information for preliminary admission decisions.
• Log referrals, prepare reports, and respond to inquiries, providing telephonic support as needed.
• Promote our care/services to prospective patients and clients.
• Receive verbal orders from physicians to initiate care/services where permitted by law.
• Collect and enter customer information into our management information system.

• Graduate of an accredited nursing school.
• Current Registered Nurse (RN) licensure in the state of Michigan.
• At least one year of experience as a Registered Nurse (RN).
• Prior home health care experience is preferred.
