In this vital position, you will:

• Deliver outstanding nursing care to patients with psychiatric and medical needs in their homes.
• Conduct comprehensive psychiatric and medical assessments, providing recommendations to the patient's medical provider.
• Collaborate closely with patients, families, and interdisciplinary teams to develop and implement effective care plans.
• Provide education on care plans, medications, therapy, diet, and exercise.
• Support family members in understanding the patient’s care and home safety.
• Ensure goals are met and coordinate discharge from services.

We’re looking for candidates who meet the following criteria:

• Active, unrestricted Registered Nurse (RN) license in the State of Michigan (BSN not required).
• At least one year of clinical experience as a Registered Nurse.
• At least one year of psychiatric experience.
• Current CPR Certification.
• Reliable transportation with a valid state driver's license and current car insurance.
