• Conduct ongoing assessments of the patient’s physical and psycho-social status
• Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a care plan
• Provide education and interventions for effective pain management and assess responses to treatment
• Ensure availability of necessary equipment, supplies, and medications
• Revise the IDG plan of care as patient/family needs change
• Document patient/family services accurately and timely
• Regularly attend IDG meetings

• Active, unrestricted Licensed Practical Nurse license in the State of Michigan
• Minimum of one (1) year of relevant LPN experience (required)
• One (1) year of management experience (preferred)
• One (1) year of home health experience (preferred)
• Current CPR Certification
• Community health and a broad range of Medical-Surgical experience (preferred)
• Reliable transportation with a valid state driver's license and current car insurance
