- Conduct ongoing assessments of the patient’s physical and psycho-social status
- Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a care plan
- Provide education and interventions for effective pain management and assess responses to treatment
- Ensure availability of necessary equipment, supplies, and medications
- Revise the IDG plan of care as patient/family needs change
- Document patient/family services accurately and timely
- Regularly attend IDG meetings
- Active, unrestricted Licensed Practical Nurse license in the State of Michigan
- Minimum of one (1) year of relevant LPN experience (required)
- One (1) year of management experience (preferred)
- One (1) year of home health experience (preferred)
- Current CPR Certification
- Community health and a broad range of Medical-Surgical experience (preferred)
- Reliable transportation with a valid state driver's license and current car insurance
