Overview
The Assistant Supervisor primary responsibilities include:
Provides oversight, support and care for consumers of Hope Network’s service in a variety of settings; role model appropriate behavior and skills to consumers to help them achieve their highest level of independence; use a consumer’s prescribed plan(s) in support of their medical, clinical, therapeutic and behavioral needs. Provide leadership and direction to the Independent Living Instructors and the Lead Independent Living Instructors as appropriate and when the Residential Supervisor is not present.
Facilitate consumer’s medical appointments, distribute medication scripts and update appropriate medical/ medication paperwork.
- Regular and predictable attendance is an essential requirement of this position.
- Complete weekly CLP service schedules based on overall consumers’ needs and hours of availability. Also, provide this information to the Residential Supervisor and the CLP Independent Living Instructors (and the CLP Administrative Coordinator when applicable).
- Schedule medical appointments for consumers that participate in the program (in conjunction with the CLP Administrative Coordinator when applicable).
- Assist with collection of data for Periodic Reviews, predicted outcomes, and Global Outcomes as requested by the Residential Supervisor (or the CLP Administrative Coordinator when applicable).
- Assist with intake packet completion with new CLP consumers and ensure information is accurate and complete. Distribute information to appropriate team members.
- Assist the Residential Supervisor (and the CLP Administrative Coordinator when applicable) with the completion of program documentation including health and medical records, Medication Administration Records, portable medical profiles, and home and community-based support system assessment.
- Participate in the on-call rotation as assigned.
- Associate degree preferred in a medical field with three years’ experience in a related setting.
- Demonstrated critical thinking/problem solving skills to respond appropriately to medical and behavioral issues.
- Demonstrated leadership skills and ability to problem solve independently.
- Demonstrated ability to train, orientate and mentor staff.
- Demonstrated ability to take initiative and action to respond, resolve and follow up regarding service issues with consumers/family members/guardians in a timely and appropriate manner.
- Current, valid state driver’s license and acceptable driving record according to Hope Network policies.
- At least two years of supervisory experience preferred.
- At least two years working in a TBI setting is preferred.
