Overview
The Population Health RN – Risk Adjustment Specialist will play a critical role in improving clinical documentation for Hierarchical Condition Category (HCC) coding accuracy across a portfolio of primary care practices. This position bridges clinical expertise with health technology to ensure documentation supports accurate risk adjustment, quality reporting, and optimal patient care outcomes.
Scope of WorkServes as a population health subject matter expert in the practice. Plans and prioritizes care for individuals and a population of patients, focusing on strategies that promote optimal health within populations and directs patients to the right care team member at the right time. Demonstrates expertise and current knowledge in nursing care and management of a sub population of patients of varying complexity, seeking to improve patient, family and health system/community outcomes through education, skills, and preventive care in a managed care environment.
Qualifications- Bachelor s Degree or equivalent in nursing or a healthcare related field (Required)
- 3 years of relevant experience in clinical nursing and/or case management/managed care or related field (Required)
- Licensed Registered Nurse (RN) – State of Michigan (Upon Hire) (Required)
- CRT-BLS (AHA Ambulatory 90 days) or CRT-BLS (ARC Ambulatory 90 days) (Required)
- Experience in Primary Care or Population Health (Preferred)
- Familiarity with EHR systems and health technology platforms (Preferred)
- Proficiency in Microsoft Excel (pivot tables, charts, data analysis) (Preferred)
- Strong presentation and communication skills (Preferred)
- Demonstrated ability to manage multiple practices and priorities (Preferred)
- Knowledge of HCC coding, risk adjustment, and value-based care model (Preferred)
- Change management experience (Preferred)
