We are seeking a Case Manager who will be responsible for person-centered planning to develop an Individual Plan of Service. Provide ongoing linking, coordinating, and monitoring of services.
Job DetailsThis position is 40 hours a week, Monday through Friday, 8:00 am to 5:00 pm. There are no weekend, on call, or holiday requirements for this position. The employee in this position will work a hybrid schedule, working in office at 790 Fuller Ave NE Grand Rapids, Michigan 49503 and within the Kent County community.
CompensationPay: $21.99 – $29.15. Final job offer is based on several factors (internal equity, skills, education, experience, and credentials).

- Bachelor’s degree in a human services related field (e.g., Social Work, Education, Psychology, Sociology).
- Six months of full-time experience working with persons with mental illness, substance abuse disorder, or intellectual or developmental disabilities (fieldwork and/or internship experience may apply).
- Possession of a valid driver’s license and reliable transportation for job-related use.
- Strong organizational skills, with experience in person-centered planning.
- Demonstrated understanding of individuals with severe mental illness and substance use disorders.
- Demonstrated communication skills that effectively convey information.
- Demonstrated active listening.
- Demonstrated time management skills.
- Licensed BSW.
- Knowledge of trauma-informed theories, principles, and practices.
- Lived experiences with mental illness, developmental disabilities, or substance use disorders valued.
