Hours:
8 - Shift: First Shift (United States of America)
- Assesses and prioritizes patient/client treatment needs, including discharge needs.
- Implements plan of care and collaborates with the clinical team to assure a uniform plan of care.
- Provides individual, group, and family therapy or interventions; conducts psychosocial assessment, including assessment of co‑occurring disorders.
- Establishes therapeutic relationship with assigned patients/clients, engaging, deescalating, screening, and managing crises.
- Serves as a liaison between community resources, family, patient/client, and hospital‑or‑clinic‑based providers of care.
- Monitors length of stay based on continual reassessment of patient’s progress towards goals.
- Reports potential risks to patient safety to the manager, attending physician, or appropriate team leader.
- Models the mission and values of the organization, presenting a professional image in appearance, attitudes, and behaviors, time management, and communication skills.
- Master’s degree in social work, LLMSW, or LMSW.
- Current Michigan licensure in social work.
- One year of mental health experience to independently assess patient needs, implement and evaluate effective plans of care, and utilize knowledge of policies and procedures.
- Preferred two years post‑graduate experience in substance use disorder treatment and additional experience in mental health.
