Overview

The Unaccompanied Children (UC) Transitional Foster Care (TFC) Lead Medical Coordinator is primarily responsible for overseeing health services as well as coordinating the provision of timely, appropriate health services for children in care. This position is responsible for developing and maintaining policies and procedures for confidential health record management, triage of health concerns, medical emergency response, environmental health and safety, and post-release health care planning/care coordination.

The Lead Medical Coordinator maintains accurate and timely medical records, appointments, and services in accordance with the requirements of the program. This position is responsible for partnering with other health professionals regarding health and safety standards for out-of-home childcare, childcare licensing requirements, disease reporting requirements for care providers, immunizations of children, medication management. This position maintains knowledge of community health and mental health resources for children.

This position must provide child-facing services on-site.

Annual Salary Range $61,200.00 - $72,000.00

  • Maintains accurate and timely medical records, appointments, and services in accordance with the requirements of the program.
  • Partners with other health professionals regarding health and safety standards for out-of-home childcare, childcare licensing requirements, disease reporting requirements for care providers, immunizations of children, medication management.
  • Schedules, coordinates, and follows through on the medical and dental services provided to clients.
  • Sets up and maintains the medical record for each program client.
  • Ensures the initial medical screening, TB screening, immunization records, and medical recommendations are included in the medical record.
  • Conducts frequent concurrent audits to ensure data entry and medical records are accurate and correct.
  • Addresses specific medical conditions with medical providers and resources, while serving as a liaison with the provider and foster family.
  • Assists foster parents and case managers in addressing any barriers that interfere with timely medical services for clients.
  • Assists the program with communication medical needs, conditions, recommended treatment, and medical emergencies to Office of Refugee Resettlement (ORR).
  • Maintains accurate, up-to-date information on clients in care while meeting ORR and CCWM's documentation and reporting requirements.
  • Stays abreast of all agency, federal, and state regulatory requirements related to social services.
  • Transports clients on an as needed basis.
  • Provide child-facing services on-site.
  • Drives for agency business.
  • Performs other special assignments at the supervisor’s request.
