Scope of Work
  • Plans and prioritizes care for individuals and populations of patients, focusing on strategies that promote optimal health within populations.
  • Provides expertise and current knowledge in nursing care and management of caseloads of patients of varying complexity, seeking to improve patient, family, and community outcomes through educational concepts, skills, and preventive care in a managed care environment.
  • Assesses internal and external referrals to identify patient needs, level of intensity, insurance benefits, and other resources.
  • Develops care plans and recommends strategies to primary care providers, specialists, and other health‑care team members to maximize continuity of care across the continuum.
  • Assesses the educational needs of patients, families, and team members; develops and implements appropriate teaching strategies or makes referrals as needed.
  • Communicates and collaborates with patients, significant others, providers, and payers to coordinate services that improve access to appropriate care and promote cost‑effective health outcomes.
  • Measures patient care outcomes, interprets reports, and analyzes data trends for groups of patients.
  • Documents patient data, interventions, and outcomes according to department guidelines.
Qualifications
  • Bachelor's Degree in Nursing or a healthcare‑related field (BSN required within 7 years for hires on or after January 1, 2017).
    BSN must be obtained within 7 years for those who start in this RN role on or after January 1, 2017 (Spectrum Health Ludington Hospital).
  • 2 years of relevant experience in clinical nursing and/or case management/managed care or related field (Required).
  • 5 years of relevant experience in clinical nursing and/or case management/managed care or related field (Preferred).
  • Registered Nurse (RN) license in the State of Michigan (Required upon hire).
  • Case Manager Certification (CCM) preferred; CCMC Commission for Case Manager Certification (Preferred upon hire).
