Home Health Case Manager

The PACE Home Health Case Manager RN promotes and maintains the health of participants in the community through teaching, counseling, and coordination of appropriate preventive and rehabilitative services with the PACE interdisciplinary team. The Home Health Case Manager RN is responsible for assessment of health needs and oversight of the plan of care, initial implementation of nursing care plans and providing nursing care, as well as periodic re-evaluation of individual and family nursing needs.

This role provides technical, organizational, and interpersonal skills necessary to coordinate the scheduling, communication, and encounter documentation of all aspects of the plan efficiently and effectively for home care. The Home Health Case Manager RN oversees and updates the CNA plan of care. Who Should Apply? Pay and Benefits:

Must have unrestricted RN license in the State of Colorado Must have valid driver's license, reliable transportation and auto insurance coverage Cell phone and laptop provided for documentation
