Looking for a weekday nursing role with predictable hours, meaningful patient relationships, and supportive leadership?

At Summit West Care, our LPNs work regular daytime schedules, provide one‑on‑one patient care, and are treated as trusted professionals—not just task coverage.

We’re a mission‑driven, non‑profit home health agency serving Mesa County, focused on ethical care, realistic workloads, and long‑term sustainability for our staff.

• Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans

• Perform nursing visits including assessments, treatments, and follow‑up care

• Administer medications and treatments within LPN scope of practice

• Monitor patient condition and report changes to the supervising RN or care team

• Educate patients and caregivers on medications, treatments, and safety

• Complete accurate, timely documentation in the EMR with training and support provided

• Collaborate with RNs, therapists, caregivers, and leadership to support coordinated care

• Travel locally within Mesa County (mileage and drive time reimbursed)

• Current, unrestricted Colorado LPN license

• Ability to work independently in a home‑based setting

• Strong clinical judgment and communication skills

• Comfort with documentation and basic technology (EMR training provided)

• Valid driver's license, reliable vehicle, and auto insurance

• Home health experience preferred (but not required— we provide training)

Stable, Predictable Schedule

• Monday–Friday, 8:00 AM – 5:00 PM

• No nights, weekends, or on‑call rotations
