In this role, you'll deliver one-on-one skilled nursing, manage your own daily schedule, and focus on what matters most: supporting patients' health, safety, and independence at home.
No floor ratios.
No assembly-line visits.
Just skilled nursing, done well.
- True skilled nursing practice (SOC, recertifications, assessments, care coordination)
- Autonomy, flexibility, and trust in your clinical judgment
- A non-profit organization where patient care comes before profit
- Provide skilled nursing services in patients' homes
- Complete Start of Care, recertification, and routine skilled visits
- Perform comprehensive assessments, medication management, wound care, and patient education
- Coordinate care with physicians, therapists, caregivers, and families
- Document accurately and efficiently using Homecare Homebase (HCHB) EMR
- Participate in interdisciplinary care planning and collaboration
- Travel locally within Mesa County (mileage and drive time reimbursed)
- Active Colorado Registered Nurse (RN) license (required)
- Home health or skilled nursing experience preferred (training available)
- Strong clinical judgment and comfort working independently
- Solid time-management, communication, and documentation skills
- Familiarity with Medicare-skilled services preferred
- Reliable vehicle, valid driver's license, and auto insurance
- A commitment to ethical, patient-centered nursing care
