OverviewAveanna Healthcare is seeking a Clinical Director to join our compassionate team in Grand Junction. Must have an RN license in good standing.
Position OverviewThe Clinical Director is responsible for the planning, organizing, directing, and evaluating nursing services of the location (non‑PDHC offices). The Director manages all operational services, works within federal, state, and regional regulations covering home health, and collaborates closely with the Area Clinical Director to establish and implement program development. The nursing team reports to the Director who oversees hiring, discipline, training, and all nursing‑related items.
Salary$88,000–91,000 annual salary + quarterly incentive potential up to 10% of salary dependent on office success.

- Assign patients for day‑to‑day management to nursing coordinators and/or Nursing Supervisors/Managers
- Maintain open lines of communication with families and physicians
- Evaluate new referrals for admission with nursing care recommendations
- Monitor clinical status of all patients and ensure coordination of patient care
- Ensure quality and appropriateness of services
- Monitor staffing patterns of all patients
- Ensure compliance with Nurse Practice Act
- Evaluate and supervise staff based on job‑description goals
- Ensure RN supervisors/clinical care managers are aware of updated policies and procedures
- Support and assist in registry nurse hiring, orientation, and education PRN
- Maintain awareness of third‑party payer rules and regulations
- Assist and support marketing efforts
- Maintain environmental budget awareness
- Maintain effective interdepartmental communication
- Perform direct patient care when needed
- Participate in location administrative and clinical call as needed
- Perform on‑site supervisory visits to assess client, family, environment, and clinical care givers; complete follow‑up documentation
- Registered Nurse, Associate/Bachelor's Degree or equivalent experience
- Current Nursing License
- One year of management experience
- Clinical or other work experience as per state and/or federal laws and regulations
- Valid Driver’s License and acceptable MVR
- One to two years of pediatric/neonatal experience preferred
- One year of home care experience preferred
- Adhere to confidentiality standards and professional boundaries at all times
- Knowledge of compliance with regulations
- Comfortably work with families with limited resources
- Quick‑thinking and astute decision‑making skills
- Attention to detail, time management, problem‑solving, conflict resolution
- Strong commitment to clinical excellence, leadership skills, ability to train and supervise staff
- Effective organization and communication skills
- Speak, write, read, and understand English
- Travel as required; company does not provide vehicles or transportation
- Lifting, carrying, pushing, and pulling 25 lbs; lift 50 lbs; prolonged walking, standing, bending, kneeling, reaching, twisting
- Sit and climb stairs; visual and hearing acuity; strong sense of smell and touch; reposition patients and move equipment without assistance; respond physically and mentally to emergencies
- Occasional rapid movement and physical agility (e.g., children, neurological patients)
- Perform duties in an office environment during agency operating hours
- Expose to allergens and other conditions; possible exposure to blood, bodily fluids, and infectious diseases
