In addition to having strong clinical skills, Hospice Nurses are “teachers” – helping patients identify their own personal goals of care and helping caregivers have confidence in caring for their family member. They are generalists on all diseases and specialists in symptom management, requiring excellence in assessment and critical thinking.

The Admission RN will work a schedule of 4x10s (8a- 6p) rotating through weekends (about 2 every 6 weeks).

SUMMARY OF JOB RESPONSIBILITIES
: The Admission RN is responsible for providing information about Hope West services and community resources to individuals with serious illnesses. This role involves assisting patients and their families in determining which Hope West service line best meets their needs and determining their eligibility for those services. As the primary point of contact for hospital discharge planners, the Admission RN coordinates and completes the referral and admission process in various settings, including home, hospital, and facility environments.

QUALIFICATIONS
:

• Valid and current RN license in the state of Colorado maintained throughout employment.
• Graduated from an accredited school of nursing;
  Bachelor’s degree preferred.
• Hospice experience required
• Must have or obtain hospice certification (CHPN) by the end of the third year of employment.
• Valid Colorado driver’s license, unlimited access to reliable transportation, and adequate automobile insurance maintained throughout employment.
• Able to work in a variety of environments.

COMPETENCIES & REQUIREMENTS
:

• Ability to set priorities and follow through.
• Demonstrates clinical excellence in assessing patient/family situation.
• Able to participate in the on-call, holiday, weekend, and after-hours programs.
• Current CPR certification is maintained throughout employment.
• Communicates effectively, both written and verbal, with a variety of customers
• Adequate computer skills preferably with Microsoft office products.

PAY & BENEFITS:

• Starting hourly pay: $35.00 - $39.50
• Generous weekend differential of $5/hr
• $1/hr each for BSN/CHPN/WCC
